How to register

Birth

For direct registration
In case of births in hospitals/clinics/institutions, a medical report provided by the concerned medical officer along with the duly filled -in birth registration form  is to be submitted to the Registrar. And if the birth took place at home, then a certificate is to be issued by the mid-wife concerned along with the duly filled -in birth registration form  is to be submitted to the Registrar.

For online registration (through web)
In case of births in hospitals/clinics/institutions, a medical report provided by the concerned medical officer is to be submitted to the Registrar at the time of extraction of birth certificate or collection of birth certificate. And if the birth took place at home, then a certificate is to be issued by the mid-wife concerned  is to be submitted to the Registrar at the time of extraction of birth certificate or collection of birth certificate.



Death

For direct registration
If the death occurred in a hospital/clinic a medical certificate of cause of death issued by the hospital/clinic is to be sent to the Registrar along with the duly filled-in Death Registration form.

For online registration (through web) 
If the death occurred in a hospital/clinic a medical certificate of cause of death issued by the hospital/clinic is to be sent to the Registrar at the time of extraction of death certificate or collection of death certificate.
